At Lion City Chiropractic, we are dedicated to providing you with quality care and transparent communication. That’s why we want to share our policies and procedures with you.
Appointments & Scheduling
- Appointments should be pre-scheduled; missed appointments are the responsibility of the client to reschedule.
- All programs are time-based and have a maximum number of adjustments associated with them.
- All missed appointments must be made up before your program expiration date. If total adjustments are utilized earlier, the expiration date will be modified accordingly.
Payments
- Payment plans are automatically recurring to the card on file, as per the option chosen.
- If payment is declined, the account is considered delinquent, and payment will be required before your next scheduled adjustment. (This is regardless of whether you have appointments to be made up or not; no payment, no service.)
Program Termination
If you choose to terminate your program of care, here are your options:
All plans are non-refundable; however, if you choose to transfer your unutilized program, it will be calculated as follows: prepaid amount minus all services rendered.
All services utilized from the date of purchase will be charged at our prevailing retail rates.
